Consent Form

    I, Angelica Carvalho, am doing a research project for my Research on language and literacy class. The main focus of my study is Medical in a discourse community: The interaction between a physician (expertise) and patient (compliance). The participants for this study involves a 58 year old woman with a medical condition, a physician that has been following her condition and possibly a nurse, who has assisted her with her medical necessities. The names of these participants will be kept anonymous and confidential.

This study will consist in a period of two to three days about thirty to forty minutes per day if allowed. With the permission of my participants, video or audio recording will be part of my research study. In my research project I will conduct interviews and an ethnographic study. This research study is voluntary; therefore, each of my participants can withdrawal from the study at any time.

Interviews, ethnographic study and video or audio recordings will be disposed of once the study has ended. The privacy of this research study will be read only by my fellow classmates as well as my professor, who is also my Faculty advisor.

There are no benefits within this research study, as well as there are no financial obligation on the participants behalf. Within this research study there will be no compensation for participation. Once again my research study is focusing on Medical in a discourse community: The interaction between a physician (expertise) and patient (compliance). Once again, my participants have the right to withdrawal from this study at any time.
